A continence assessment is a detailed assessment of how a participant’s bladder and bowel are working, of any issues the participant has getting to, or using, the toilet, to work out if the participant has incontinence and what type of incontinence they have.
There may be different factors that can cause problems with passing urine (wee) and faeces (poo). Some participant’s continence problems may be resolved with the right treatment, however, for some participant it may only be managed by consumables.
A continence assessment can be attended face to face or by Telehealth. Part of the assessment can be completed in just one visit or some complex situation may need follow up visit to complete the assessment part.
Any continence experienced Registered Nurse/Clinical Nurse and Continence Nurse can conducts a thorough assessment and provide management plans and a comprehensive list of recommendations of consumables.
There are many parts included in the comprehensive Continence Assessment and report.
Assessment part includes basic information on participant, health, history, diagnosis, medications, current situation/problem and bowel and bladder profile. Participant, their family, support workers, their general practitioner (GP), specialists or other health care professional, such a physiotherapist and occupational therapist can help with information to complete the assessment part.
Recommendations and management plan is then developed by a nurse from the above assessment. These can be direction on schedule toileting, consumables to use, referls to specialists etc.
Finally, the report is completed with comprehensive list of tables of consumables that are continence products to help with managing incontinence issues.
Continence: for participants who need daily assistance with toileting (bladder and bowel).
In case of Support workers, family members, or participants themselves need education and training around continence and a careplan, our nurses are available and experienced to provide the training.
A participant may be in need or an intermittent catheter to void urine or maybe limited mobility and suffers from incontinence of urine/ faeces or both. There is a high risk of skin integrity compromise. Our nurses can provide a bedside training and also provide with management CarePlan.
